Over 50% of site coordinators use online reporting and ordering. If you're not one of them, give us a call at 1-800-397-3263 or email us at admin@raisingreaders.org. We'll set up your username and password and show you how to use the system – on the phone, or in person. While the online system is the best way to report and order, we understand that some site coordinators do not have access to the internet. For those sites the following fax forms are still available:

Who participates? All Maine hospitals who provide birthing services, midwives who attend home and birth center births, and virtually all Maine pediatric providers who see children for well child care are enrolled in raising readers.

Who's eligible to participate? Primary care providers who render well child care to children from birth to age five within the state of Maine. If your office is NOT enrolled, download an application here: Health Care Site Application or call us at 1-800-397-3263.
